The NHS App is designed to notify you when a new message arrives, but you will only receive these alerts if notifications are enabled on your device. For instructions on how enable these, please click here.

Without notifications switched on, you may not realise that a message has been sent to you unless you open the app and check manually.

Change your personal details

Use this service to change your address, name or phone number. It is important that we have up to date contact details in case we need to contact you.

You can use this service if you:

  • are registered at the surgery

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
  • evidence if your name has changed due to marriage or by deed poll

Are you moving?

Please check our catchment area to ensure that your new home is within our catchment area. We do agree to keep some patients moving out of our catchment area but this is decided on a case by case basis e.g. if working locally.

It is not a right and patients accepted for this form of out of area registration would not be entitled to home visits. Also, an out of area registration is not suitable for patients who need other services arranged geographically such as district nurses, health visitors, and mental health services.
