Including ethnicity in your health record

Use this service to inform the surgery of your ethnicity so we can make sure your ethnicity is included in your medical record.

You can use this service if you:

  • are registered at the surgery

Information about your race and ethnicity helps us make sure we provide the highest quality of care for all patients. Studies show that our racial and ethnic backgrounds may place us at different risks for certain diseases.

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
