Raised blood pressure reading review

Use this service to submit a review of your raised blood pressure.

You can use this service if you:

  • are registered at the surgery
  • have been invited to do so

We are contacting you as you have had a raised blood pressure (BP) reading recorded in the past few years which has not been reviewed. A high reading can be caused by a number of things such as stress, or even rushing to your appointment. However, we do not want to miss hypertension.

Managing this early can prevent problems (e.g. heart attacks or types of dementia).

You can check your blood pressure by using a machine that you have access to, by using our blood pressure ‘Pods’ (walk in and ask at reception) or by asking your local pharmacist.

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of

Take a minimum of two readings, leaving at least a minute between each. Write down the lower of the two. If the first two readings are very different, take 2 or 3 further readings and write the lowest.
